Introduction to Health Surveys

Health Surveys is where you can fill out questionnaires provided to you by your healthcare provider or researcher.

Your health surveys is the first section to appear on the home screen. On the home screen, You will be able to see which surveys you are currently working on.

To view all your surveys, click on "View All". 

This section will include all your current health surveys, repeating health surveys and completed health surveys. While filling out a questionnaire, all answers will be saved, should you pause and exit the Health Storylines app. 

You also have the ability to export and save a copy of the completed survey.
